Play Ace and King of trumps (LHO drops 10 and Q) and then a low heart to dummy. LHO must duck, and RHO will then be out of hearts.
Cash dummy's last diamond and exit with a trump. If RHO has the 13th diamond, let him cash it while you toss a heart. Either way he will then be forced to lead a club giving you two heart discards and 10 tricks.
