Curious end position

Here's a position I came across as declarer, South, with hearts trump and the lead in dummy.
How to take three of the last five tricks?
If you try to pitch a club on the "good" diamond West will ruff, exit with a black card and must come to two more trump tricks. Finessing clubs does not work either.
Instead you must ruff a spade, cash the ♣A and exit with a club.
The real test of a good declarer, though, is knowing that this is the position!
